Dear Residents,

With spring approaching, it’s time to have your HVAC units inspected. With use, HVAC units become clogged, which can cause serious leaks and damage to your unit and possibly units below. Please note that damage caused by a leaking HVAC unit is the responsibility of the unit owner where the leak originated. The Association will be conducting these inspections May 16^h through May 20^th and May 23^rd through May 27^th. With your permission, staff will enter your unit, replace old filters with new*, clear out any clogged tubes and empty drain pans. If you prefer to use your own HVAC contractor, indicate below and let the office know that this service has been completed by May 27, 2011.

If you choose to use The Association staff, indicate below by marking which week you prefer. If you will not be home to allow staff access, please drop off keys to the management office during normal business hours. Once this form is completed, please drop off at either the front desk or the management office.

(All pets must be restrained while this work is being completed)  

I, __________________________, owner of Unit# ________ will be using a HVAC contractor of my choice.  I will drop off paperwork to The Management Office verifying that this maintenance was completed prior to May 27, 2011.

*Filters will be charged at cost.
